United States Renal Data System

This is "a national data system that collects, analyzes, and distributes information about end-stage renal disease (ESRD) in the United States." Features the Annual Data Report, which (as of 2008), "includes a volume dedicated to the Chronic Kidney Disease population, a first for public health surveillance of CKD in the United States."
